The mission of the Haak’u Health Center Pharmacy Department is to provide safe, effective medications that support the highest quality care for our patients.

Meet The Pharmacy Team

You can call to refill your prescription up to 24 hours in advance* 

In order to make your refill, we’ll need:

· The patient’s name

· Patient’s chart number or date of birth

· Patient’s next appointment date

· A list of ALL the medications that need to be refilled

· A telephone number where staff can reach you if they have questions.

*Some prescriptions cannot be refilled without a doctor’s visit.

Understanding how to take your medicine is key to having it work!

· ASK your doctor or pharmacist to go over how to take your medicine.

· CALL the clinic if your medicine’s side effects are bothering you.

· DON’T STOP taking your medicine without talking to your doctor.

· DON’T SHARE your medicines with friends or family members.

· CHECK with the pharmacist if your pills look different with a new prescription.

· BRING your medicines and glucometer to every clinic visit.
